Vocational Specialist

Federation of Organizations is looking for a full-time Vocational Specialist to join our Forensic Assertive Community Treatment (FACT) team/Assertive Community Treatment (ACT) team. This individual will supervise and provide direct services, with a focus on vocational and educational services, to individuals with severe and persistent mental illness. The Vocational Specialist serves as a member of a mobile, multi-disciplinary treatment team, delivering individualized care to those in need.

QUALIFICATIONS:

  • Master’s degree or higher in Social Work, Psychology, or Rehabilitation Counseling (if Rehabilitation Counselor, C.R.C. preferred)
  • Minimum 1 year of experience in a treatment or rehabilitation setting
  • A valid and clean NY State driver license
  • Fingerprinting, criminal record check
  • Approval from NYS Office of Mental Health
  • Strong documentation and organizational skills
  • Effective time management skills
  • Knowledge of treatment and rehabilitation methods for persons with severe and persistent mental illness
  • Skilled with individual and group counseling techniques

CORE & ESSENTIAL RESPONSIBILITY:

  • Maintain case records and assume responsibility for caseload assigned by the Team Leader
  • Manage referrals and intake assessments in accordance to agency timeline and at the discretion of the Team Leader 
  • Identify individualized plan based on participant’s vocational and educational goals, immediate needs, and readiness
  • Offer support for vocational and educational services to participants with severe mental illness  
  • Conduct daily visits in the community or based on requirements from the Team Leader
  • Collaborate, complete, and review all updates at a minimum of every 6 months including Treatment Plan Review, Comprehensive Assessment Update, Complete Wellness Plan, and more
  • Complete weekly progress updates
  • Complete visits based on requirements from Team Leader
  • Prepare and submit reports by deadlines
  • Perform other duties consistent with participant’s goals and as directed by the Team Leader

SALARY: $50,000.00 per year
