JOB TITLE:               Therapist, Wyandanch Clinic 

REPORTS TO:           Program Manager

DEPARTMENT:        Clinical

CLASSIFICATION:   Non-Exempt                       

HOURS:                      35 Hours

QUALIFICATIONS:            

Masters in Social Work or Mental Health Counseling or related field. NYS license required (LCSW, LMSW, LMHC, LCAT, LMFT). A valid and clean NY State driver license, fingerprinting, criminal record check, and approval from NYS Office of Mental Health.

JOB FUNCTION:

Provide therapeutic individual, group and family interventions to assist members in growth and recovery. Work as part of an interdisciplinary team. Complete all required documentation, including treatment plans, progress notes, assessments, etc. Comply with program and OMH regulations.
