Federation of Organizations is looking for a full-time Social Worker or Mental Health Counselor for the Home-Based Services Program.

CORE & ESSENTIAL RESPONSIBILITY:
Exciting full-time job opportunity for LMSW or LMHC to join the Home-BEST Program through Nassau County SPOA (Single Point of Access) services. Candidates can be hired without their license as long as they are working towards obtaining licensure. This is a clinical role and will count towards clinical hours for LCSW. The role of this position is to work with high-risk children/ adolescents aged 5-21 in their homes and communities to provide extra support to prevent hospitalizations or residential placement. The candidate will be a part of a multidisciplinary team consisting of a Care Coordinator, Youth Peer Advocate, and Family Peer Advocate. The team is responsible for collaborating with the client’s treatment providers to ensure the translation of therapeutic skills into the home and linkage to community resources. The Home-BEST Program only services families in Nassau County. The candidate will provide weekly in-home sessions to a caseload of 12 clients. As home visits are required, the candidate needs to have a clean driver’s license and mode of transportation. Mileage reimbursement is offered.
Salary: $27.47 – $28.03 per hour
