JOB TITLE: Care Coordinator
LOCATION: Remote (Staff are expected to report to West Babylon for meetings & supervisions)

DEPARTMENT: Care Coordination
HOURS: 2 positions available one Part Time & one Full Time
Bachelor of Arts or Science with two years of relevant experience, or a license as a Registered Nurse with two years of relevant experience, or a master’s with one year of relevant experience serving children and adults with serious mental illness. Clean, valid NYS Driver’s License preferred. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
The Care Coordinator will assist families in navigating the mental health service system-including ability to make referrals to mental health and community resources, crisis intervention/ diversion and peer support services. Services include but not limited to: (1) Providing education and linking of the behavioral health managed care benefit package (2) Collaborates with inpatient staff and MCO (as applicable) to affect successful transitions out of inpatient or institutional settings (3) Addressing the quality, adequacy and continuity of services to ensure appropriate support for individuals’ mental health and psychosocial needs (4) Completing plans of care and coordinating with the Department of Social Services and Social Security Administration. The Care Coordinator will minimally collaborate with the family once a week, with at least one face-to-face visit per month. The other weekly contacts can be face-to-face or via phone, depending on the family’s situation. The number of visits and calls could increase based on the family’s needs. If the youth is in residential placement, the Care Coordinator will continue to work with the family when the youth is home during visits. They will ensure that the service plan created by the residential care coordination team is implemented. Oversees and coordinates access to all services (behavioral, medical health and social service supports) to assist children and families to stay healthy in Nassau County Only. Manage a caseload size of approximately 9-27 individuals and assist with other duties as needed. Completes all required documentation according to standards and no later than 48 hours of service delivery.
