Care Coordinator (Suffolk State Aid)
REPORTS TO: Program Supervisor
DEPARTMENT: Care Coordination
CLASSIFICATION: Non-exempt
HOURS: 35 Hours per week
QUALIFICATIONS:
Minimum of a Bachelor’s Degree with no experience serving people with mental illness, developmental disabilities or alcoholism or substance abuse. However, experience is preferred. Clean, valid NYS Driver’s License preferred. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
JOB FUNCTION:
Provide care coordination and health promotion to individuals with severe mental illness and/or 2 or more chronic health conditions to avoid costly inpatient hospitalizations, emergency room visits and unnecessary care. Manage a caseload size of approximately 25 individuals and assist with other duties as needed. Oversees and coordinates access to all services (behavioral, medical health and social service supports) to assist individuals to stay healthy. Completes all required documentation according to standards and no later than 24 hours of service delivery.
