JOB TITLE: Care Coordinator (Floater)
LOCATION: Floater position between Richmond Hill (Queens) and Long Island
REPORTS TO: Program Supervisor
DEPARTMENT: Care Coordination
HOURS: 35 hours per week
Minimum of a Bachelor’s Degree and 2 years of relevant experience serving people with mental illness, developmental disabilities or alcoholism or substance abuse. Clean, valid NYS Driver’s License preferred. Fingerprinting, criminal record check, approval from NYS Office of Mental Health. Spanish speaking preferred.
Provide care coordination and health promotion to individuals throughout Long Island and NYC (Bronx, Queens, Brooklyn, and Manhattan). This position requires floating (as needed) between the locations and or working on special projects. Provide Care Coordination to those with severe mental illness and/or 2 or more chronic health conditions to avoid costly inpatient hospitalizations, emergency room visits and unnecessary care. Assist individuals in obtaining Food Stamp and Social Security benefits. Oversees and coordinates access to all services (behavioral, medical health and social service supports) to assist individuals to stay healthy. Manage a caseload size of approximately 50-60 individuals or a specialized caseload of 12-15 individuals and assist with other duties as needed. Completes all required documentation according to standards and no later than 24 hours of service delivery.
