Dina Vagelatos, MSW
Dina Vagelatos, MSW has worked for Federation of Organizations for over 15 years, where she served as a Program Manager for Case Management and Care Coordination throughout Long Island and Queens. Ms. Vagelatos now serves as the Associate Director of Care Coordination, and is charged with managing several Health Home Programs throughout NYC. Ms. Vagelatos is responsible for ensuring that quality services are provided to the members served in her programs so they may continue on the road to wellness. With her wealth of experience and vast knowledge of resources, Ms. Vagelatos is focused on connecting as many people as possible to these important services. Ms. Vagelatos has a Bachelor of Science degree in Psychology from Sacred Heart University and a Master’s of Social Work from the State University of New York at Stony Brook.
