Federation of Organizations Clinical Services Department serves over 1,400 individuals at a time through Personalized Recovery Oriented Services (PROS) Programs, Outpatient Mental Health and Substance Use Clinics, and Assertive Community Treatment (ACT) Programs across Queens Kings and Suffolk Counties.
Federation of Organizations is now offering outpatient addiction services for adults and children/adolescents. Services are designed to help each person gain support and maintain sobriety. Individuals will learn the effects substance use has on the body, mind and interpersonal relationships and will learn how to break the cycle of physical and psychological addiction. Family members involved will gain an understanding of addiction and develop skills to support their loved ones and take care of themselves

Federation currently has three (3) ACT teams, located in Suffolk and Queens County, providing assertive outreach and treatment to individuals with serious mental illness. Federation brings together a mobile interdisciplinary team, which includes a psychiatrist, psychiatric nurse practitioner, registered nurse, social worker, employment specialist, substance abuse specialist, and family specialist and peer specialist. The purpose of the ACT team is to provide comprehensive services to individuals who are diagnosed with severe mental illness and whose needs have not been met by traditional service delivery approaches. Services are offered to individuals in their homes, in the community or wherever they would like to meet. The ACT team supports recovery through a highly individualized approach. Services include a full range of clinical treatment, psychological rehabilitation and community support services designed to promote recovery, improve psychiatric symptoms, prevent relapse, teach skills, provide assistance and secure community-based resources necessary to be successful in work, school, home and social relationships.
In addition to ACT services, Federation also provides Forensic Assertive Community Treatment (FACT) services in Queens County. Utilizing the success of the ACT model, the FACT mobile multi-disciplinary team provides services to individuals who are diagnosed with severe mental illness whose needs have not been met by more traditional service delivery approaches and who also possess a criminal history background. The FACT team aims to serve individuals with forensic histories, substance abuse issues, difficulties with medication and treatment non-compliance, are under AOT orders, lack support networks, have a history of homelessness, and have educational and vocational needs. The goal of the FACT team is to provide traditional ACT services as well as specialized services to reduce criminogenic behaviors and recurring involvement with the criminal justice system.

Federation of Organizations operates two P.R.O.S. programs, Recovery Concepts West in Copiague and Recovery Concepts in Patchogue. Recovery Concepts is a comprehensive program that brings together rehabilitation support and clinic services, all under one roof. Recovery Concepts offers wellness classes, individual attention and intensive support to avoid relapse from mental health symptoms as well as substances. The program provides assistance to members who are working toward their own personal goals, including employment and education. Services are provided both on-site as well as in the community. Family, friends and loved ones are welcome to be involved in each participant’s treatment.
Participants develop an Individualized Recovery Plan (IRP) – a course of care built on the individual’s strengths, personal ambitions and hopes. The IRP is done in partnership with staff and can include anyone the member feels will help in supporting their recovery. Individuals can pick and choose which services are right for them.
The services provided include the following:
Community Rehabilitation and Support (CRS)
These services are designed to help develop the skills and supports necessary for each individual to achieve wellness. Services are designed to focus on strengths in order to manage mental and physical health, develop needed skills, supports and resources so the individual can live successfully in the community.
Ongoing Rehabilitation and Support (ORS)
ORS services are designed to assist individuals in maintaining employment. These services include vocational counseling, advocacy and support while on the job. ORS services can take place at the work place or outside of work hours, depending on what works best for each individual.
Intensive Rehabilitation (IR)
These services are designed to help the individual attain specific life goals, such as returning to school, obtaining employment, securing volunteer work or finding a new place to live. They may also be used to provide targeted interventions to reduce the risk of hospitalization or relapse, loss of housing or involvement with the criminal justice system, and to help individuals manage their symptoms. Once a specific goal is established, our dedicated and experienced professionals will assist the individual in achieving their goal.
Clinical Services
These services help each person to manage their mental health condition so they can pursue their personal goals. They include clinical counseling, medication management, health assessment, symptom monitoring and treatment for substance abuse.
Integrated Dual Disorder Treatment (IDDT)
Integrated treatment for dual disorders is designed to address the mental health and substance abuse needs of persons with co-occurring disorders simultaneously. Such service includes, but is not limited to, motivational interviewing, cognitive-behavioral and harm reduction approaches. When necessary, practitioners coordinate care with appropriate substance abuse providers when it is determined that the co-occurring disorder is acute or serious and the individual is ready to accept related treatment.
Community Based Services:
Services under this umbrella include Adult and Children’s Home and Community Based Services (HCBS), Community Empowerment and Recovery Services (CORE), and the Children and Family Treatment and Support Services (CFTSS) program. Through these programs, eligible participants can receive clinical and support services in their homes or other community-based settings. Services are tailored to the unique recovery goals of the client and can include assistance with accessing community resources, employment and education support, and clinical care for mental health and substance use disorders.
