HARP Outreach/Engagement Specialist
LOCATION: (Remote- Within LI & NYC)
REPORTS TO: Program Supervisor
DEPARTMENT: Care Coordination/Outreach
HOURS: Up to 35 Hours per week
Minimum of a high school diploma. Spanish speaking or multi-lingual very helpful, personal experience/family experience with recovery preferred. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
Provide outreach via telephone and in person to individuals meeting the criteria for HCBS services. Work to engage participants over the phone and get them interested in HCBS. Schedule appointments for assessors to conduct field visits and place follow up calls to ensure participants were satisfied with services. Conduct follow up calls and internet searches to find individuals who cannot be located via phone numbers or addresses. Document all outreach efforts in an electronic health record. Contact medical providers as needed for the purposes of outreach and assist care coordinators in finding resources in the community that can assist those they are coming in contact with.
