LOCATION: Queens

REPORTS TO: Senior Case Management Specialist                           

DEPARTMENT: Care Coordination

CLASSIFICATION: Non-Exempt                         

HOURS: Full time or Part time

 

QUALIFICATIONS:              

Minimum of High School Diploma, Bachelor’s degree preferred. Personal lived experience in recovery. Peer Specialist Certification from the Academy of Peer Services preferred, Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

 

JOB FUNCTION:

To assist people in recovery from mental illness residing in adult homes as well as those who moved from the adult home to reach their personal recovery goals. Assists individuals to develop independent living skills, connect with community resources as well as self-help and other natural supports. Facilitate groups to help individuals develop skills and prepare for independent living. Acts as a role model for residents by sharing personal experiences of wellness and recovery.
