Adult Home Plus Care Coordinator
LOCATION: Remote – Reports to Richmond Hill for supervisions and meetings
REPORTS TO: Program Manager
HOURS: 35 Hours per week
4 years’ experience serving people with mental illness and a Bachelor’s Degree in a related field or Master’s Degree in a related field with 2 years’ experience. Clean, valid NYS Driver’s License preferred. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
Serves as the single dedicated care manager for individuals with serious mental illness transitioning from adult home settings to supported housing. Provides intensive care management throughout the transition process, from In-reach, to assessment to transition and ongoing. Is responsible and accountable for coordinating all aspects of the individuals’ care. Serves as the single point of contact between the individual and various healthcare and community based providers, housing contractors, family members, adult home operators, guardians and other supports involved with the person. Links the person to appropriate services and supports in the community. Works to decrease avoidable hospitalizations and emergency department visits and promotes the health and well-being as well as the successful transition to supported housing for those served.
