Clinical Social Worker- Fee for Service

Location: Wyandanch- Clinic

Job Function: Provide direct clinical care to clients as needed as a member of a multi-disciplinary treatment.

Qualifications: LMSW credential. LCSW preferred. At least 1 year of training and/or experience in a clinical setting.  Bi-lingual/Spanish speaking is preferred. Registered Nurse, RN

Provide management and administration of medications, in conjunction with the psychiatrist, as a member of a mobile, multidisciplinary treatment team for persons with severe mental illness and forensic involvement.

Psychiatric Nurse Practitioner (PT)

Location: Copiague & West Babylon

Job Function: Provide psychiatric and medical assessment and treatment as a member of a multi-disciplinary treatment team within the clinical services department for persons with severe and persistent mental illness.

Team Specialists (FACT)

Location: Queens & Suffolk County

Job Function: Lead responsibility for integrating peer and self-help information and services with the tasks of all team members, as a member of a mobile, multi-disciplinary treatment team for persons with severe mental illness and forensic involvement.

Qualifications:            Completion of a peer advocacy training program. At least one year of experience working or volunteering in a treatment or rehabilitation setting. Fingerprinting, criminal record check, and an approval from NYS Office of Mental Health.

Job Function: Provide therapeutic groups and individual interventions to assist members in growth and recovery. Responsible for engaging members in employment.

Work as part of an interdisciplinary team.  Complete all required documentation within a

48 hour period and complies with program and OMH regulations.

Outreach Services

 

Care Coordinator – Health Homes Serving Children

Location: Queens & Long Island (Medford)

Job Function: Provide care coordination and health promotion that is family-and-youth driven, supports a system of care that builds upon the strengths of the child and family.  Service individuals aged 21 and under with Serious Emotional Disturbance, Complex Trauma, and/or 2 or more chronic health conditions to integrate the delivery of physical and behavioral health services, identify and address needs early to prevent escalation and longer term need for higher end services, maintain child in community and at home with support services.  Collaborates with families, providers, and schools as well as oversees and coordinates access to all services (behavioral, medical health and social service supports) to assist individuals to stay healthy.  Complete all required documentation according to standards and no later than 24 hours of service delivery.

Qualifications: Minimum of a Bachelor’s degree  with at least 2 years relevant experience; OR Registered Nurse with 2 years relevant experience; OR Master’s Degree with 1 year relevant experience. Advocate (Part time)

Location Patchogue

Job Function: Responsible for representing Federation with interfacing agencies at meetings and events and advocates for the needs of people in recovery from mental illness within the mental health system.  Provide peer support, advocacy, education and information to staff, volunteers and program members. Facilitate groups at designated hospitals.

Care Coordinators (Various shifts within NYC & LI Region)

Job Function: Provide care coordination and health promotion to individuals with severe mental illness and/or have 2 or more chronic health conditions. This will include linkage to behavioral, medical, residential, social, financial, and vocational resources; collateral contacts with both behavioral and medical treatment follow up with referral and treatment appointments, and education of health promotion and preventive care. The care manager will communicate with the treating clinicians to discuss enrollee’s care needs, conflicting treatments, change in condition, etc. which may necessitate treatment change (i.e., written orders and/or prescriptions).

Qualifications: Minimum of a Bachelor’s degree or a CASAC with at least 2 to 5 years of experience serving people with mental illness, developmental disabilities or alcoholism or substance abuse.  Clean, valid NYS Driver’s License preferred.
