Clinical Social Worker- Fee for Service

Location: Wyandanch- Clinic

Job Function: Provide direct clinical care to clients as needed as a member of a multi-disciplinary treatment. 

Qualifications: LMSW credential. LCSW preferred. At least 1 year of training and/or experience in a clinical setting.  Bi-lingual/Spanish speaking is preferredExperience conducting group and individual therapy. Experience providing family psycho-education and/or other family support services.  Experience working in an interdisciplinary team. Clean, valid NYS Driver’s License. Fingerprinting, criminal record check, and an approval from NYS Office of Mental Health.

Registered Nurse, RN

Location: Copaigue & Wyandanch

Provide management and administration of medications, in conjunction with the psychiatrist, as a member of a mobile, multidisciplinary treatment team for persons with severe mental illness and forensic involvement. 

Qualifications: Currently licensed as a registered professional nurse by the New York State Education Department. Clean, valid NY State Driver’s License. Minimum of one year of experience in a treatment or rehabilitation setting. Fingerprinting, criminal record check, and an approval from NYS Office of Mental Health.

Team Specialists (FACT)

Job FunctionLead responsibility for integrating peer and self-help information and services with the tasks of all team members, as a member of a mobile, multi-disciplinary treatment team for persons with severe mental illness and forensic involvement.

Qualifications:Completion of a peer advocacy training program. At least one year of experience working or volunteering in a treatment or rehabilitation setting. Fingerprinting, criminal record check, and an approval from NYS Office of Mental Health.

Psychiatrist- Per diem, PT, FT

Location: Ozone Park

Job FunctionProvide psychiatric and medical assessment and treatment as a member of a mobile, multi-disciplinary treatment team and in a clinic setting for persons with severe and persistent mental illness.

Rehabilitation Counselor

Location: Patchogue- PROS

Job FunctionAct as a guide or coach to help people in recovery from mental illness who are served by the PROS program to follow their own path to recovery.  Provide information, training and hands on assistance in skills building to individual members and groups. Provide peer support to members of the program.  Complete required documentation and complies with program regulations.

Substance Abuse Specialist

Location: Copiague (PROS)

Job Function: Provide therapeutic groups and individual interventions to assist members in growth and recovery. Responsible for substance abuse treatment at program.

Work as part of an interdisciplinary team.  Complete all required documentation within a 

48 hour period and complies with program and OMH regulations.       

Qualifications: CASAC preferred. Masters in Social Work or Mental Health Counseling or related field preferred. NYS licensed preferred. At least one year of experience in mental health and substance abuse treatment. Valid and clean NYS Driver’s License, fingerprinting, criminal record check, and an approval from NYS Office of Mental Health.

Outreach Services

Advocate (Part time)

Location Patchogue

Job Function: Responsible for representing Federation with interfacing agencies at meetings and events and advocates for the needs of people in recovery from mental illness within the mental health system.  Provide peer support, advocacy, education and information to staff, volunteers and program members. Facilitate groups at designated hospitals.

Qualifications: High School Diploma or equivalent.  Individual must be in recovery from a behavioral health condition. Clean, valid NYS Driver’s License.  Peer Certification preferred. Fingerprinting, criminal record check and all necessary clearances from the Justice Center.

Care Coordinators

Location: Medford- Care Coordination- Nassau & Suffolk Regions

Ozone Park- Care Cooridination- Queens, Brooklyn, Bronx, Manhattan & Suffolk Regions

Job FunctionProvide care coordination and health promotion to individuals with severe mental illness and/or have 2 or more chronic health conditions. This will include linkage to behavioral, medical, residential, social, financial, and vocational resources; collateral contacts with both behavioral and medical treatment follow up with referral and treatment appointments, and education of health promotion and preventive care. The care manager will communicate with the treating clinicians to discuss enrollee’s care needs, conflicting treatments, change in condition, etc. which may necessitate treatment change (i.e., written orders and/or prescriptions). 

Qualifications: Minimum of a Bachelor’s degree or a CASAC with at least 2 to 5 years of experience serving people with mental illness, developmental disabilities or alcoholism or substance abuse.  Clean, valid NYS Driver’s License preferred.
